I'm glad you like the idea of medic-nin Kakashi. I picture him as very much still considering himself a field-shinobi, actually, but there are *many* reasons that him learning the skills are necessary for this time period, (not the least of which is the fact that his *students* may need healing, and he's the best they've got in this time after his self training.)
As to why he's not acting as a traveling doctor... while he is bound to do a bit of that while he takes missions, he's actually got *other* plans and obligations that will be taking up the last half of the year he's waiting out. (You'll get to read what those will be in the next chapter, probably posting this Friday.) And yeah, there's no denying his placement as a main line Hatake, so the time-travel truth will need to be shared with the clan head. (Again though, more on that in the next chapters.)
However, the Uchiha eye seal is actually only a protection against bloodline thieves, it doesn't actually shut it off. (You may be thinking about the eye seal mentioned in the "Kakashi and the Shikabane-hime" 'verse, actually, as that one does both). Though Shikako may eventually upgrade his seal in this 'verse so that it does that *too*. However, the fact that he has properly activated the mangekyou *does* mean that it's less of a drain on him at this time, compared to if this had all gone down before the DoS Hospital Arc.
Kakashi and already-returned-to-the-future-Sasuke-and-Shikako made *plans* for how to justify Kakashi having a Sharingan in this time, (most of it boils down to claiming Kakashi as their teacher after the Uchiha clan had so drastically failed them... and Kakashi losing an eye while saving them from bloodline thieves who'd *already* procured a sharingan. Shikako then implanted the eye in Kakashi. The Shikako and Sasuke in the past will totally support this story... and given the fact that he now knows healing jutsu and the fact that their fighting style will be reflective of his, the teacher story will hold up well.)
It's even more fun to think of the unintentionally political conundrum the whole thing could be seen as, since it's essentially like Kakashi adopted them and raised them, as seen in this time period where apprenticeships is more like an adoption, because you only teach family.
Lol- Yes! Thattle be a thing that they absolutely need to sort out between the clans. Shikako, in particular, is ridiculously entangled, if you consider Sasuke's intention to get her acknowledged as a Nara again too.
