After the events of the true pacifist route in Undertale Red & Yellow (with Frisk choosing to stay with Toriel):
Monsterkind has been living peacefully on the surface among humans ever since a young child, Frisk, saved them from their long captivity in the underground.
All is well for Frisk and their friends. All except Clover, a young boy who was recently brought back to life by Asriel Dreemurr as gift for his selfless sacrifice in the underground. While he appreciates his second chance at life, he has never gotten over a friend he wishes he had never had to say goodbye to. Chara Dreemurr, the first human to fall into the underground, and one of two ghosts, alongside Clover, who guided Frisk through their journey, has not forgotten about her friend either. Unlike Clover, Chara was not given a second chance at life, and continues to watch over Frisk on the surface. While she enjoys Frisk’s company, Clover has always been at the forefront of her mind. Unknowingly to Clover, there is one way for Chara to return to the living realm. However, Chara has been hesitant to act on this.
After all, such an act would require a permanent marriage of souls.

But a human, no older than 10 fell underground, and as luck would have it, the human was taken by the royal guards, when the human was to be executed by Asgore, however the Human's SOUL was damaged, extremely damaged, so Asgore greenlit Project Determination, which formerly experimented on monsters with DETERMINATION, but reached a dead end, until the human was brought to them. The head of the Project, Dr W.D Gaster, The creator of The CORE, hired two other scientists to assist him in this project, Dr.Alphys and Dr.Chujin.
Or a universe where a human fell after Toriel left the royal family and instead was experimented on.

Will she be able to save these siblings?
TLDR: After a fatal mistake, Chara and Clover’s souls become bonded to one another.
